Requirements:  Bachelor's Degree in Social Work, Psychology or related field.  Experience doing outreach with the SMI/MICA population, knowledge and awareness of homeless and community services available. 
Responsibilities:  Participate in a multi-disciplinary treatment team.  Engage all consumers in treatment planning and all areas of service delivery.  Provide crisis intervention.  Assist consumers with ADLs through modeling, direct assistance and coaching. 
Status:  Full time with benefits
