LICENSED PRACTICAL NURSE

Requirements:  High school diploma or equivalent.  Graduate of an LPN program.  One year mental health experience preferred but not required.  New Jersey LPN license required.
Responsibilities:  Provides case management services to medicaiton only consumers which includes referrals to external social service and health care resources.  Completes discharge summary of medication for medication only consumers.  Serves as consumer advocate.  Assists psychiatrists and APN's to maintain appropriate documentation in the medical chart.  Conducts reviews of medical records.  Communicates with consumers primary care provider and documents findings.  Calls in psychiatrist and APN's order for consumers psychotropic medication to pharmacies.

CREST CASE MANAGER
